To apply for a donation, please follow the instructions below:

Once these forms are received at the corporate office, we will review each request within two weeks.

All requests must include the Total Wine donation request form, a cover letter, and proof of 501(c)3 status from the IRS sent to donations@totalwine.com to be considered; we can no longer accept donation requests via fax, mail, or phone.

Please note that a 60-day advance notice is required for all requests.
